The chocolates you describe do not sound familiar. Are you based in the UK? Most of us who post here are British-based. It's a lovely idea though.

Yup in the UK, it wasn't a normal brand i.e. cadburys but a square flat ish box.

Hmm, I've heard of personalised chocolate (mrs chocadmin just bought a couple of bars), but not questionnaire based.

Are we talking a box or a bar? The bloomsbury bars (found on this site and available in sainsburys) have a wide variety of amusing wrappers.
Personalised or 'fun' wrappers are the kind of thing you find at findagift.co.uk or prezzy box.

Thanks but no it's not them, not sure why but they've reversed the photo on the trivia bag and they're out of stock.
The box was a bit like your milk tray box with a tray of chocolates, I'm going to spend a chunk of weekend if I can get away from the other half trying to find them.
