I think Baby Smurf is a boy, as Smurfette is the only girl (until they create little Sasette to keep her company). I'm not sure why baby is so pale; perhaps some extra smurfberries are needed in his diet? Very rich in iron!

Interestingly, in the cartoon Baby Smurf wears white, but on most merchandise he wears pink.
